What is in the index, and what is not
The Academy writes its own diagnostics for the problems staff actually describe, and it imports the official catalogues rather than reproducing them from memory. Where a catalogue has not yet been imported, this page says so. A plausible-looking list of rule identifiers that we had invented would be worse than an empty one.

A funded learner has no eligibility evidence on file
Funding has been claimed for the learner, but the college cannot produce the evidence that they were eligible for it. The claim may be entirely correct and still fail, because at audit the evidence is the claim.
Work it throughA large number of validation errors appeared suddenly
Validation has produced a large number of failures that were not there before. A sudden change in volume almost always has one cause rather than many.
Work it throughA learner is flagged on a data quality or funding monitoring report
A record meets the criteria of a report that identifies data worth checking. That is a request to investigate, not a statement that the record is wrong. Some flagged records are entirely correct and simply need their evidence to be locatable.
Work it throughA learner left weeks ago but is still showing as continuing
The learner has stopped attending, but the ILR still shows their aim as open with no actual end date, so the system believes they are still in learning and keeps generating funding for them.
Work it throughAn adult learner appears to be funded by the wrong authority
The learner's recorded source of funding does not match what their home postcode implies, so the aim may be claimed from the wrong funder, under the wrong rules.

The aim has an actual end date, so the system knows it finished, but nothing records whether the learner achieved it. The achievement rate calculation treats an aim with no achievement recorded as a non-achievement.
Work it throughAn aim is on the ILR but generates no funding
The aim validates and appears on the ILR, but generates no earnings. Validation and funding are different tests: a record can be perfectly valid and still not qualify for funding.
Work it throughAn apprentice is on the ILR but is not generating any payments
The apprentice exists on the ILR and looks correct, but no earnings are being generated for them. Apprenticeship funding depends on the ILR and the apprenticeship service agreeing, so a record that is perfect in one and absent or different in the other produces nothing.
Work it throughFunding dropped this month and nobody can explain why
The funding report for this period shows less than expected, or less than last period. Because every return restates the whole year, a change made this month can move earnings reported in earlier months, so the drop may have nothing to do with this month's delivery.

A learning support claim has been made, but one of the links in the chain is missing: the assessment of need, the record of support actually delivered, the cost of delivering it, or the recording that triggers the claim.

The planned hours recorded for 16 to 19 students do not reconcile to the timetable being delivered, so students may be in the wrong funding band.

The learner has stopped, and the question is whether to record a break in learning, which pauses the programme on the basis that they will return, or a withdrawal, which ends it. The two have different funding and achievement consequences, so the choice is not administrative.
Work it throughThe achievement rate is lower than the college expected
The published or modelled achievement rate is lower than the college's own view of how well learners did. The difference is usually in the data rather than in the teaching.
Work it throughThe same learner appears twice
One person has been recorded twice, usually with two different learner reference numbers, and sometimes with two different ULNs. The system treats them as two people.
